The Physical Examination and Medical History forms are not submitted online. A paper copy of both forms must be submitted to an Athletic Trainer or Head Coach to consider the physical complete. A copy of these forms may be found under the Download and Print tab.

OrthoArkansas HIPAA Form: The Bryant Public School District has a contract with Athletic Trainers and Team Physicians who are employed by OrthoArkansas. This form in no way binds you as a patient to OrthoArkansas or allows them to share your information with a 3rd party entity. It simply allows those employees to speak to your athletes' coaches and administrators regarding any athletic injury or illness your athlete may sustain.


We would like to thank you for taking part in our new process of completing athletic participation paperwork online. This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.

• To access the online forms hold your curser over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

• Click on the form name and fill out the information requested. You must have your student’s ID number available, as it is required on each form.

• To sign the document, click inside the signature box and hold your mouse over the box. This will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click the refresh icon next to the signature box.

DO NOT USE ATHLETE NICKNAMES ON FORMS.

To access a physical copy, click the arrow on the Download/Print tab to print off a blank copy. Take this with you to your child’s physical appointment and after completion please return to the Athletic Trainer or Athletic Office.
